VOLUNTEER AGREEMENT AND RELEASE OF LIABILITY FORM (one form per person)

Medical/Release of Liability

I agree to sign this form electronically. I am a volunteer, not an employee, with Calvary Disaster Relief (aka "CDR", a ministry of Calvary Chapel La Habra) and will be participating in ANY disaster relief work between January 1, 2025 to December 31, 2025. I understand that reasonable safety precautions will be taken by the leaders of this ministry and any other organizations, companies, or individuals participating with Calvary Disaster Relief. I also understand that unforeseen hazards do exist. I hereby assume all risk of injury, harm, damage, or death in connection with my participation. I understand and agree that neither Calvary Disaster Relief, Calvary Chapel La Habra, nor its trustees, representatives, employees, agents, or volunteers shall be held liable in any way for any occurrence or incident which may result in injury, harm, damage, or death. I hereby release Calvary Disaster Relief and Calvary Chapel La Habra, its trustees, employees, agents, and representatives as well as any other organizations, companies, or individuals working in conjunction with Calvary Disaster Relief, from any injury, harm, damage, or death which may occur while participating in this ministry. I also hereby release the recipient property owner, whether real or personal, from any injury, harm, damage, or death which may occur while rendering services as a volunteer of Calvary Disaster Relief. To the fullest extent permitted by law, I agree to hold harmless the above listed parties from any claim by myself, my estate, successors, assigns, or other persons. I further state that I am authorized to sign this document; that I understand the terms herein are contractual and not merely recitals; and that I have signed this document of my own free will and volition. I further acknowledge that I have fully informed myself concerning this release by reading it before I signed it. I hereby consent to the provision or procurement by Calvary Disaster Relief of emergency medical care or first aid in the event that I suffer any illness or accident while performing volunteer activities. I hereby release, discharge, and hold Calvary Disaster Relief harmless from any claim related to the provision of such emergency care. I acknowledge that CDR is a Christian organization, and I will conduct myself in a manner consistent with the biblical values outlined on Calvary La Habra's "Statement of Faith" webpage. I assume responsibility to familiarize myself with these expectations. I acknowledge that any photos or videos taken of me by CDR/CCLH and its actors/volunteers that may be used for promotional purposes, and I consent to having pictures/videos of me taken and posted on any social media platforms.

Minor Volunteers

Medical/Release of Liability

I agree to sign this form electronically. Minors under the age of 16 are not permitted to volunteer with Calvary Disaster Relief. If I am a minor between 16 and 18 years of age, I understand that I can perform volunteer activities only under the supervision of a parent/legal guardian, or designated adult. In that case, this release must be signed on my behalf by such parent or legal guardian, and I will not volunteer without such action.
